An application for registration of a de facto adoption shall be made to the Registrar in the form in Schedule 1.

“Registrar” means a Registrar of Adoptions appointed under section 4 and includes a Deputy Registrar;
The Registrar shall register a de facto adoption by entering the particulars thereof in the Register.
