The Registrar shall remove from the register the name and other particulars of a registered pharmacist who-

"Registrar" means the Registrar of Pharmacists referred to in section 5. Establishment and constitution of Board.
has died;
has failed to make an application for the retention of his name on the register before the expiry of the year in which the name is -
registered under subsection (2) of section 8;
retained in the register under subsection (2) of section 14; or jiii)
restored to the register under subsection (1) of section 18;
