Where the name of a person has been removed from the register under paragraph (cj of subsection (ll of section 16, the Board shall on receipt of the required information, restore the name to the register upon application in such form as the Board may determine and on payment of the fee as prescribed in the
register
"register" means the register of pharmacists maintained in pursuance of section 6;

Where a name removed from the register under paragraph (bj or (cf of subsection (1) of section 16 has been restored, such restoration shall, if the Board so directs, have effect from the date of the removal of the name.
